The client opt-in form collects your name, email, optional phone number, service interest, availability, message, and permission choices. Please do not include sensitive medical, financial, or identity information.
How your request is handled
FormSubmit processes the form and forwards the information to onexisalon@gmail.com so One XI Salon can respond, discuss services, and coordinate scheduling. FormSubmit may use spam protection and retain submissions under its own policies. FormSubmit privacy policy.
Client feedback
The feedback form collects your contact details, visit and location information, selected concern categories, comments, requested resolution, and follow-up permission. Feedback is processed through FormSubmit and emailed directly to Catherine Michele at onexisalon@gmail.com for review and response. It does not enroll you in marketing. Please do not provide sensitive personal information or private medical details about yourself or anyone else.
Stylist applications
Stylist applications collect contact details, career and license information, work history, preferences, availability, consent, and any résumé or portfolio files you choose to upload. FormSubmit processes applications and forwards them directly to Catherine Michele at onexisalon@gmail.com for review and follow-up. You may separately opt in to having your application kept for future opportunities. Contact Catherine to withdraw that permission or request an update or deletion of your information.
Your choices
Consent to receive occasional marketing emails is optional and separate from permission to respond to your inquiry. You can withdraw your marketing permission by emailing onexisalon@gmail.com. The form does not enroll you in automated text marketing.
